An Infertility Procedure Can Help You Have a Baby

The most successful infertility procedure is in vitro fertilization, or IVF. This involves using drugs to stimulate the ovaries and retrieve multiple eggs, then fertilizing them with sperm in the lab. Then an embryo is transferred into the uterus.
In IVF, women take ovary-stimulating medication for 8 to 12 days. This causes them to produce several eggs instead of just one each month. They also have regular transvaginal ultrasounds and blood tests to monitor their ovaries and hormone levels. During the egg retrieval phase of an IVF cycle, doctors use a small needle to remove eggs from the woman’s body. The doctor will also test the eggs to make sure they are mature.
An embryo is fertilized when a sperm and an egg join to form a single cell called a blastocyst. The blastocyst must go through some major hurdles in order to be ready for transfer to the uterus. Laboratory staff will monitor the blastocyst for five to six days to make sure it is progressing normally. Some fertilized eggs may not develop into a blastocyst, and these are typically frozen for future use.
A healthy blastocyst should stick to (implant) in the lining of the uterus and grow. But if it doesn’t, the doctor will use another procedure to help it stick. Known as assisted hatching, the procedure uses warm fluid to open part of an embryo’s outer covering, which increases its chances of attaching to the uterus.
Other procedures in IVF include intracytoplasmic sperm injection (ICSI), which involves injecting a single healthy sperm into an immature egg. This is often used when the sperm quality or quantity is poor, or when IVF fails to work.
Another ART procedure is intrauterine insemination (IUI). In this procedure, the doctor places sperm directly into the uterus through a thin tube (catheter) during ovulation. This increases the chances that sperm and an egg will fuse to form a blastocyst, leading to pregnancy.
Other ART procedures include zygote intrauterine insemination (ZII) and tubal intercourse with sperm. ZII involves placing sperm inside the fallopian tubes to increase the chances that it will meet an egg. With tubal intercourse with sperm, a catheter is placed in the fallopian tube and filled with saline or air bubbles during a hysteroscopy or saline sonohysterogram (SIS). This allows the provider to see whether the tubes are blocked and if they are in the correct position for a successful implantation. This is done in the office and is usually painless.
